This form is your first step to working with us at Bridging the Gap. Just let us know a bit about yourself and what you’re looking for, and we’ll help you set up your New Client Assessment with Seth. Intake Waiver

This waiver is just a simple agreement for a one-time, at-home exercise consultation. It explains that the session is for general guidance only (not medical care) and asks you to take responsibility for your own safety. By signing, you’re agreeing to be careful, understand the risks, and not hold the specialist responsible for accidents that might happen during the visit.

Client Service Agreement

This agreement lays out how your personal training and massage sessions with Bridging the Gap, LLC will work. You’ll set your session price at your intake appointment, and we'll send the official contract through DocuSign in the first month, so everything’s handled securely and online. The agreement covers things like payments, scheduling, cancellations, and privacy, making sure we’re both protected and everything is transparent. If you have any questions, we're always happy to help!

  1. Complete the intake form above with your contact information and service needs.
  2. Call 949.346.4488 or email us to schedule an introductory appointment and discuss how we can best support you.
  3. Seth Hodosh will coordinate your ongoing session times based on current scheduling availability.
  4. When you receive your first month’s invoice, you’ll also be sent a DocuSign form to review and sign the Client Liability Waiver and confirm your individualized pricing.
